In the last fifteen years the surgical treatment of breast cancer has received revolutionary innovations by consequent to new knowledge on the natural history of these tumors. The impossibility of control in these disease by extended surgery, checkups revealing smaller tumors and the willingness of "todays patient" to participate in the choice of treatment, have created conditions for conservative procedure. The results of retrospective and especially prospective studies on conservative treatment have demonstrated that this combined therapy can give adequate local control of the tumor The Milan trial 1, comparing Halsted mastectomy versus QUART has given an important contribution which shows no difference in terms of overal/survival, disease-free survivaI and local relapses between the two treatments.
