Giant Retroperitoneal Liposarcoma

Introduction: Although uncommon, mesenchymal neoplasias may constitute a therapeutic challenge when they hit major proportions. The aim of the authors is to describe the case of a peritoneal liposarcoma that, due to its large volume, presented peculiarities in its therapeutic planning, as well as to review its clinical presentation, diagnosis and treatment. Case Report: A 52-years-old male patient presented complaints of increased abdominal circumference and a large palpable mass. Computed tomography and magnetic resonance of the abdomen revealed a large retroperitoneal tumor with displacement of several intra-abdominal viscera, suggestive of voluminous malignant tumor from retroperitoneal origin. The patient underwent laparotomy with resection of the mass together with the right kidney. Pathological examination revealed that it was a dedifferentiated liposarcoma measuring 62 cm in the greatest diameter, weighing 19 kg. Complementary treatment with radiotherapy was indicated. There was an excellent postoperative outcome, with total resolution of complaints, once the patient remained asymptomatic 36 months after the procedure. Conclusion: Retroperitoneal liposarcomas are rare clinical cases, with variable clinical presentation according to their location, histological type, malignancy and size. The possibility of cure still is the radical surgical resection with clear margins. Once the patient presents good clinical conditions to tolerate the procedure, voluminous size is not necessarily a contraindication to surgical treatment.
