Abstract

A divided segment of ileum and colon is used to create a neobladder after cystectomy with a cecourethral anastomosis. This surgical technique for creation of the “Mainz Pouch" uses 15 cm of cecum and ascending colon and two ileal loops of the same length for construction of na urinary reservoir. Such a bladder provides volitional voiding with continence and a large low pressure reservoir with na antireflux ureteral anastomosis. Four patients underwent a Mainz Pouch procedure for total bladder substitution after a standard nerve-sparing cystoprostatectomy. Follow-up ranged from eight to 14 months. The four patients have continence and they are completely dry day and night. Three patients empty the neobladder at normal intervals spontaneousiy. One patient has erections and sexualintercourses.
