Integração de Dados Atuais dos Linfossarcomas na Classificação da OMS, Seu Valor Para Previsão do Prognóstico e Adaptação da Terapêutica do Prognóstico

Abstract

A identificação dos linfossarcomas com os marcadores imunológicos enriqueceu o valor prognóstico da Classificação OMS. O prolinfocítico (centrofolicular) tipo B e o subtipo linfoblástico nulo* têm um bom prognóstico enquanto que o tipo Imunoblástico (T ou B) e o subtipo linfoblástico T têm um prognóstico pobre. Muitas classificações de Hematossarcomas ou Linfossarcomas malignos Não Hodgkin foram propostas (3, 9, 16, 17, 22, 23, 29). Algumas ainda são usadas, principalmente aquela de Rappaport (29), que foi concebida antes do estabelecimento do conceito da transformação de linfócitos pelo antígeno(s) em células grandes pironinofílicas, as quais eram confundidas com "histiócitos" antes desse conceito e foram chamadas Imunoblastos por Dameschek (7). A recente utilização de marcadores imunológicos e/ou citoquímicos capazes de reconhecer os linfócitos T e B e os assim chamados nulos (Tabela I) (6, 13, 31, 36, 38) não somente retiraram dos assim chamados tumores "histiocíticos" o Linfossarcoma Imunoblástico (20), mas também a Micose Fungóide identificada como um Linfossarcoma T. (18).
