Infecção durante a Aplasia no Tratamento de Indução das Leucemias Agudas Mieloblásticas

Resumen

Este estudo confirma a freqüência das infecções em pacientes neutropênicos, bem como ressalva que nem sempre e possível se chegar a um diagnóstico de infecção com exame bacteriológico comprobatório. A grande maioria das infecções é devida a germes Gram-negativos. Sua freqüência aumenta com a duração da hospitalização. Os exames bacteriológicos iniciais negativos são sem valor. No entanto existe, sobretudo em crianças, um certo número de infecções por germes Gram-positivos onde os exames iniciais do rinofaringe eram positivos. Este estudo confirma sobretudo que:  existe ligação direta entre leucopenia e infecção;  a idade é importante para avaliação do prognóstico, considerando-se alto risco aqueles pacientes com mais de 41 anos;  as infecções mais graves são aquelas de localização pulmonar, principalmente quando resultantes de germes como Serratia, Pseudomonas, Aspergilius;  os programas profiláticos, Instituídos antes da fase neutropênica mostraram baixo índice de infecção, bem como menor índice de mortalidade;  as transfusões de leucócitos completam a ação da antibloticoterapia.
