O autor faz, inicialmente, um estudo das complicações pré e pós-operatórias a que estão expostos os pacientes que têm alterações volêmicas. São analisados os mecanismos fundamentais das agressões anestésicas e cirúrgicas e suas implicações. São examinados, em seguimento, o síndrome de depleção e as alterações metabólicas do trama cirúrgico, em indivíduos normais e em depletados. O autor faz uma revisão crítica dos métodos de determinação da volemia. Apresenta o método idealizado por MOORE, calcado em estudos da composição corpórea com isótopos. Partindo deste sistema o autor tabula os volumes totais, globulares e plasmáticos empregando as cifras de hematócritos usuais em brasileiros hígidos de ambos os sexos. Comenta os métodos clássicos e faz um estudo do hematócrito em várias circunstâncias clínicas e cirúrgicas. Enfatiza os fundamentos fisiopatológicos da reposição sangüínea, baseando-se nos eventos decorrentes da perda de volume circulante. Faz uma revisão da depleção protéica e sua influência no processo de cicatrização e manutenção da homeoestase. Resume, finalmente, as condutas práticas de reposição da mas sa sangüínea e as medidas complementares pertinentes ao estado de depleção.
