Manipulação do pedículo arterial hepático: novas opções no tratamento dos tumores do fígado

O autor faz revisão das atuais condutas no tratamento dos tumores hepáticos, primitivos e metastaticos, onde as ressecções cirúrgicas não podem ser realizadas de imediato. As alternativas que tem sido descritas referem-se à manipulação do sistema arterial, que tem como finalidade aumentar o poder tumoricida da quimioterapia controlando o aporte sanqüíneo nessas lesões, como terapia isquêmica. As condutas, abordadas didaticamente, mostram os meios para sua execução, as Indicações, os fatores de prognóstico, o estadiamento cirúrgico e seu acompanhamento clinico. Exemplificamos uma conduta multidisciplinar num tumor primariamente irressecável e analisamos alguns protocolos usados. As condutas visam uma proposta para melhorar a qualidade e o tempo de sobrevida desses pacientes, cujas lesões hepáticas são atualmente, refratárias às terapias convencionais.
