A Citopatologia por Punção Aspiratória com Agulha Fina (CPAAF) Como Método Diagnóstico: um estudo de 138 casos

Estudam-se 138 casos de lesões tumorais e não tumorais pela técnica de Citopatologia por punção aspiratória com agulha fina (CPAAF) e comparam-se os resultados com os histopatológicos, com uma segurança diagnóstica de 80,4% e um erro de 4,34%, dados que se assemelham aos achados por outros autores. Na mama, com 98 casos estudados, a segurança diagnóstica correspondeu a 75,5% e o erro a 3,06%. Analisam-se os achados cito-histopatológicos segundo lesões malignas de mama por tipo histológico e segundo lesões benignas e malignas da mama. Os autores descrevem os critérios citopatológicos reafirmando o valor da CPAAF no diagnóstico pré-operatório. Explicam os motivos dos falso-positivos e falso-negativos achados e acreditam que se pode chegar a diagnósticos de quase certeza em lesões tais como fibroadenoma, carcinoma ductai, papUífero e medular de mama, mioblastoma de tecidos moles e melanoma. Assinalam as dificuldades diagnósticas em outras lesões da pele e em tireóide, analisando alguns detalhes citopatológicos em lesões de tecidos moles e de aparelho urinário. Questionam a validade do critério de escassez de material como sinônimo de benignidade e analisam o valor e significado das imagens de canibalismo e mitoses comparando as típicas e atípicas. Concluem que existem suficientes critérios para incluir a técnica definitivamente na metodologia diagnóstica da rotina hospitalar em oncologia.
