Abstract

Late diagnosis of oral cancer remains one of the most persistent challenges, reflecting structural flaws across primary care, professional training, and preventive policies. Despite the great availability of scientific knowledge on risk factors, initial signs, and screening methods, the epidemiological reality shows that most cases are still identified in late stages, when therapeutic possibilities are more complex and costly, and survival is significantly lower. This opinion article critically discusses the main causes of this scenario, arguing that the problem does not lie only in the lack of access but also in the insufficient integration between surveillance, clinical care, and health education. The analysis emphasizes the need to review healthcare models, strengthen the qualification of professionals in the front line — especially those in primary care — and broaden social communication strategies on risks and early symptoms. By highlighting historical and structural gaps, the text offers possible routes to tackle the issue more effectively, based on consistent public policies and clinical practices that value timely diagnosis as the key axis to reduce morbidity and mortality.
